Rural Literature A Tweet-Sourced List I am in the midst of writing a big, giant essay on the concept of rural literature that features women, POC, members of the LGBT community, etc. Im sick of the idea that rurality is either not important (see, Rebecca Solnits essay) or is full of white protestant men (literally everything about Trump voters, but notably Hillbilly Elegy). I spontaneously tweeted asking for favorite books of rural literature, and boy did I get a list! Here they are! I have read and loved many. I havent read many others! Some are classics, others just came out. But all of them focus on the diversity of rural (or small town) life in the United States. Some of these authors are white men, but the vast majority are not. Here we go. William Faulkner. I know. I am starting this list with a white man. But I think its important to think of Faulkner not just in his context as a Southern author, but as someone who grappled with the rural (and the ways in which it was diverse). If you are just starting out with Faulkner, may I suggest As I Lay Dying? Zora Neale Hurston. The original queen of the rural south, IMO. If you havent read Hurston, please order Their Eyes Are Watching God immediately. Hurston is one of the few writers who has crafted a rural, all-black world and been lauded for it (though thats recent). She was writing in opposition to the masculinist urban obsession with the Harlem Renaissance and it shows. Her folklore can be problematic, but it also shows the ways in which Hurston so valued rural African American culture. Flannery OConner. Nobody does grotesque like OConner. If you are new, definitely start with the short stories, though her novel Wise Blood is a favorite of mine. Jesmyn Ward is one of my favorite writers ever and was suggested to me about a million times for my essay. Shes won the National Book Award multiple times and writes beautifully about POC in rural Mississippi. If memoir is more your speed, her 2015 memoir  Men We Reaped is jarring but my favorite is her most recent novel Sing, Unburied, Sing. Louise Erdrich. One of my very favorite authors, Erdrich writes beautifully about the experience of Indigenous people in the United States, on and off the reservation. Jeannette Walls. I went back and forth about including The Glass Castle here, but if you havent read Wallss tremendous coming of age story you need to (and, bonus, its now a movie!) Affrilachia by Frank X Walker. This one is new to me, but has come super highly reviewed. Poetry focused on the experience as an African American from Appalachia. Toni Morrison. I would suggest Beloved if you have not yet read it. Ecology of a Cracker Childhood by Janisse Ray. Weedeater by Robert Gipe. The  Birds of Opulence by Crystal Wilkinson. Anything by Willa Cather. I just read My Antonia, which I loved, but The Song of the Lark has come up a lot. Annie Dillard. I am ashamed to say I have yet to read Annie Dillard, but shes next on my list. Jane Smiley. Ah, anything by Jane Smiley. The trilogy is great, as is A Thousand Acres, though its quite the bummer. bell hooks, Appalachian Elegy. More poetry about the experience of Appalachia, by none other than bell hooks. This is one of my very favorites. Barbed Wire Heart by Tess Sharpe. I just started this one, so no spoilers. Return to Sender by Julia Alvarez. YA focusing on migrant labor. Tomas Rivera And the Earth Did not Swallow Him Cottonmouths by Kelly Ford. Focuses on a queer women who gets caught up in Meth in her rural Arkansas hometown. Its brilliant. Sketches of Southern Life by Frances Harper. Cane by Jean Toomer. I finished this one in January and still havent quite wrapped my head around it. Its VERY modernist and may have gone over my head. But still, it was groundbreaking for a reason. Long Division by Kiese Laymon. Boondock Kollage by Regina Bradley In Search of Our Mothers Gardens by Alice Walker.   Beautiful on its own, but gives a lot of insight into the rediscovery of Zora Neale Hurston. Borderlands/La Frontera by Gloria Anzuldua. I just love Anzuldua so much, particularly This Bridge Called My Back, but Borderlands/La Frontera was so influential to me at 19. The Scamp bu Jennifer Pashley. I havent read it yet, but its come HIGHLY recommended. Im not quite sure if I should consider this rural, but I am including Dorothy Allisons Bastard Out of Carolina anyway, because its one of my very favorite books.  Once Upon a River by Bonnie Jo Campbell. This is clearly not everything and the books I havent written much about come highly recommended and are on my living room floor, waiting to be read, right now! Hope you guys enjoy.

Transfusion in time develops allergenic immunization. This limits the supply of compatible blood. These numbers may seem like lottery chances, but why take the chance. Ultimately there is death. Death is not a foreign outcome of blood transfusion. Transfusion related acute l ung injury or TRALI, was first reported in the early 90’s. It’s a life threading reaction following a blood transfusion. TRALI is now known to cause many deaths each year. However, experts believe that the number of death is much higher than what is reported in relation to TRALI, because many doctors are unaware of the symptoms. The cause for such a reaction is conclusive. New scientist states â€Å"The blood that causes TRALI appears to come primarily from people who have multiple transfusions. TRALI is the top reason for blood transfusion death in the world. Jehovah’s Witnesses have benefited greatly from their faithful course. Although their reason for not having blood transfusions are not because of the negative reasons that derive from it, but because of their devout belief in God and the Bible. They obey scripture such as Acts 15; 20 which states â€Å"abstain from blood† and Leviticus 7; 26 â€Å"you must not eat any blood. † Jehovah’s Witnesses respect Gods authority and has taken their stand against blood transfusions, regardless of the outcome. Over the years the tools and techniques of surgery without blood transfusion has improved greatly. One tool or technique used for surgeries with a lot of blood lost is called cell salvage. This involves recovering the blood lost by a patient, cleaning it, and putting it back into the patient. How can blood loss during surgery be lowered in order to lessen the chance for need of a blood transfusion? The key is preoperative planning for a successful bloodless surgery. The first thing to be considered is the amount of red blood cell (RBC) that will lost before a transfusion is needed. This is called the transfusion threshold. Another thing that can be done before surgery is to â€Å"increase the patients RBC mass. † (Watchtower Bible and Track Society, 2004) RBC mass can be increased by injection of iron into the patient. Also erythropoietin(EPO). If you increase the RBC mass and lower the transfusion threshold, it allows for an even greater acceptable amount of blood loss. Normothermia is a technique used to keep the patient’s body temperature during surgery. This helps keep the blood flowing properly. Managing the patient body temperature throughout the entire process reduces the surreal shock to the body which reduces the chances of incurring infection. The patient can be warmed by a thermal suit or a machine that infuses warm fluid into the body. The position can also help reduces blood loss during surgery. Local veins pressure changes depending on the field of relativity to the heart. Low pressure goes hand in hand with blood saved. Stanford University Medical Center is a pioneer in the use of bloodless surgery in neurosurgery. â€Å"Without sawing into the skull or so much as cutting the scalp, they are curing patients whose brain and spine tumors were not long ago considered a death sentences. † (Fillon, Mike 1997) These surgeries are possible with the use of Stanford University’s computer mediated stereotaxis radio surgery known as the Cyber-knife. The Cyber-Knife is basically a robotic x-ray gun that shots small amounts of radiation into the tumor in a lot of different directions. This kills off the infected tissue without over exposing other parts of the body to radiation. Cyber-knife is a robotic arm that locks the radiation beam on to the tumor and constantly readjusts its aim in response to the patient’s natural small movement. To help doctors in providing treatment without blood transfusions, Jehovah’s Witnesses have developed a helpful liaison service. Presently, more than 1,400 Hospital Liaison Committees worldwide are equipped to provide doctors and researchers with medical literature from a data base of over 3,000 articles related to bloodless medicine and surgery. Not only Jehovah’s Witnesses, but all patients in general today, are less likely to be given unnecessary transfusions because of the work of the Jehovah’s Witnesses’ Hospital Liaison Committees. Some did not think that it would work fast enough to make up the amount of blood needed. A number of cases have shown how quickly EPO gets results. Pygmalion Pygmalion and My Fair Lady are a modern parallel of the story of Pygmalion, legendary sculptor and King of Cyprus, who fell in love with his own statue of Aphrodite. At his prayer, Aphrodite brought the statue to life as Galatea. George Bernard Shaw's play Pygmalion is the story of Henry Higgins, a master phonetician, and his mischievous plot to pass a common flower girl, Eliza Doolittle, off as a duchess at the Embassy Ball. In order to achieve his goal, Higgins must teach Eliza how to speak properly and how to act in upper-class society. The play looks at middle class morality and upper-class superficiality, and reflects the social ills of nineteenth century England, and attests that all people are worthy of respect and dignity. Shaw's Pygmalion is Henry Higgins, professor of phonetics, who, comes upon a homely flower-girl selling flowers in the streets, makes a wager with Colonel Pickering that in three months he can so transform her as to pass her off for a lady. To Higgins, this is but a task that he accomplishes, a wager that he wins; but in Eliza Doolittle, the flower girl, a new personality has been created. With the manners and speech of a lady, she cannot fall back into her old life, and with those ways has come an asserting will, which selects Henry Higgins, her creator, as her mate. To Higgins' dismay, he finds that his laboratory case has surged into all his life, with emotional entanglements he had not anticipated. Throughout most of civilization, people have been divided into social classes. In a lot of different especially capitalist cultures there is an upper class rich, powerful and in control. Then there was a middle class, less comfortably off than the upper class, and certainly less powerful, but respected nonetheless. At the bottom there is the lower working class making up the majority of people, rarely having the necessities of life and never considered by other classes no matter how long or hard they worked on improving their situation. In the following essay, I will discuss whether George Bernard Shaw agreed with this distinction and division of society and how he exhibited his views through his renowned play Pygmalion. Throughout the play, ladies and gentleman are continuously recognized for who they are through different factors such as how they are dressed, their manners, how they speak, morality or their money. It is however noticeable that a combination of all factors is rarely to be found. For instance Henry Higgins although well dressed, well spoken and with money, has manners which could not be characterized as genteel. Alfred Doolittle (after acquiring some money) is well dressed, has some form of manners and could be classified as rich, yet is not well spoken. Nevertheless, when the maid opens the door to him she instantly perceives that he is a gentleman. So what really does make a lady or a gentleman? Alfred Doolittle arrives at Wimpole St, in the second act, and doesn't even recognize his own daughter, Eliza, just because she has been washed and elegantly dressed. Alfred: Beg Pardon, miss. Eliza: Garn! Don't you know your own daughter? Alfred: Bly me! Its Eliza. This demonstrates that the working class was not used to washing and dressing up, which was customary for the upper class. The dissimilarity in the appearance of the upper class from the working class was so sensational that even someone who was your own flesh and blood could be naturally mistaken. This trend of depicting appearances goes right through to the end of the play, when on arrival at Mrs. Higgins' house, Doolittle is mistaken for a gentleman by the maid, merely because of the way he is dressed Higgins: Doolittle! Do you mean a dustman? Maid: Dustman! Oh no sir, a gentleman. The appearance of Doolittle is taken into main consideration when it comes to deciding what class he belongs to. The question is raised, what separates the classes really, if clothing can do so much for how someone is perceived. Apart from the way people dress, they are also defined by the way they speak. Social support is the key to reduce social isolation by promoting interaction within society. It came out that half of the health care professionals poses negative attitude towards mental disorders such as schizophrenia, depression and substance abuse regarding their da ngerousness, unpredictability and fear of communicating with patients. (Naeem, Ayub, Javed, Irfan, Haral Kingdon, 2006). Researchers have proposed several theories which describe the causes of isolation and low social support of mentally ill by society. One of them is labeling theory by Scheff in 1984. This theory proposed that public rejection is due to label such as â€Å"mentally ill† which leads to provoke negative emotions such as fear and exclusion that person labeled with mentally ill should be alarm and, therefore, be set aside from communities. (Abdullah and Brown, 2011, pp 935-948). While reviewing literature I came across several strategies to reduce social isolation in patients at an individual, group and institutional level . On individual level therapeutic use of self, enhancing spirituality and rebuilding family networks are considered valuable tools for patients. Referral to support groups should be incorporat ed in care plans. ’ The primary goal of support group is to increase member’s coping ability in face of stress, to strengthen the central core of individuals` (Perese Wolf, 2005). This will help cliet to express their feelings and desires with person going through same situation. It will increase their interpersonal relationships and coping skills and enhance feeling of togetherness and decrease uniqueness. Family education should be done to make them equal part of treatment. Mass media can be involved to convey our messages to population. Institutions should arrange semi structured sessions for the health care providers so as to increase their knowledge and make them competent enough to teach patient an their family members. During my clinical rotation, I identified and addressed the issue of social isolation at an individual and group level through nursing care process. The planning phase starts with assessment and purpose to promote social interaction. Firstly, I used therapeutic communication as a measure to build rapport with patient and gave him room for expressing and ventilation feelings through active listening. Secondly, I identified teaching needs of patients and delivered ongoing teaching on relaxation technique to make him feel comfortable since physical helath also plays a vital role in maintenance of functional health pattern I involved patient in different leisure activities such as playing indoor games and drawing to increase self esteem. While evaluating my patient I noticed that he started to participate in small activities with other patients . More over, becoming more sociable, interactive and willing to participate in different activities. This was the first time I dealt with mental illness patient with decreased social support. Initially I was resistant and scared in caring for these patients. However my perception changed when I came across different strategies to deal with such patients. I tried to remain non judgmental, sensitive and empathetic towards them and applied theoretical knowledge to clinical settings. In conclusion, decreased social support is the most challenging yet interesting domain which has many solutions. We should inculcate multidimensional approach to address this issue. If you would have asked me a year ago if there was a difference in the competency level between a nurse who has their associate degree and a nurse that has their baccalaureate degree I would have said, â€Å"No†. That answer would not have been an honest one, but rather an answer born from frustration. When I was in my first year of nursing school, our instructors informed us that all across the nation Magnet hospitals were pledging that they were only going to hire entry-level nurses who had their baccalaureate degree moving forward. That news was discouraging because I had just changed careers and was considered lucky to be in a two year associates degree program, especially since nursing schools everywhere were saturated with applicants. I did not expect to come out of school and have to jump right in to a BSN program to keep myself marketable. The American Association of Colleges of Nursing states that; â€Å"Quality patient care hinges on having a well-educated nursing workforce. (Rosseter, 2012, p. 1) Now as I look back at my three years of nursing experience, I must admit that a higher education in nursing would benefit not only my ability to find and keep a job, become a platform to continue my education but also it will benefit the way I approach planning for my patients care. I now feel that a higher education level brings with it the ability to not only understand how a patient came to be under my care, but also why. If a patient comes to my hospital with a wound to his leg and that same patient has diabetes, as a nurse with an ADN degree, I may be inclined to dress the wound as ordered and treat the diabetes as ordered without thought to why or how this happened. With a higher degree in nursing practice, I will not only have a better understanding of how to help the wound heal quickly and properly ccompanied with the proper tools, but I will also have a better understanding of the patients underlying diagnosis of diabetes and how I can help the patient understand what type of care will be given at the hospital in addition to accessed care out in society to ensure that this particular problem will not arise again. I, most likely will have a better understanding of outside resources that I can pass on to the patient before discharge so that he may better manage his overall health at home. I will hopefully be able to instill in this patient the need to educate himself on his disease so that the visits to the hospital are infrequent. The higher the education level, the more confident the nursing diagnoses are. In other words, as a nurse with a baccalaureate degree, I feel I will know how to articulate in a more productive way to ensure that the patient gets the best explanation of what interventions are being performed and why. With a higher education, I should feel more confident in my abilities to perform any and all tasks needed on a daily basis to help my patients. I am very fortunate to have nurses in my work place that value education and think along the same lines as Grand Canyon University when it stated in its Nursing Philosophy, â€Å"Students are educated to provide, direct and evaluate client-centered care while focusing on the person as an integrated whole. † (Grand Canyon University [GCU], 2011, 9). One huge advantage to becoming a highly educated nurse is that I will feel more comfortable when speaking to multidisciplinary teams about patient care. I am currently a charge nurse on the diabetic floor of my hospital and I work with these people every Tuesday to discuss patient flow. I can see the level of confidence they have in their roles having been educated at a higher level than myself and I cannot wait until I can feel that same level of confidence as my co-workers. In conclusion, those of us that are continuing our education should encourage other nurses to advance their careers and education level as well. We all can benefit from higher education and many studies have been done to prove this theory. It makes sense that if we are left in charge of a patient’s well-being, we should know, with confidence that we are doing our job to the best of our ability.
